The Emma platform of Medicine Men helps patients to manage their illness and connects them to their healthcare professionals and caregivers. With a wide range of connected home measuring equipment, questionnaires and signalling functionalities, the Emma Platform provides hospitals, GP's or other healthcare organisations with a comprehensive IT infrastructure for remote care.
The Emma Platform is designed to provide healthcare professionals with the right tools to support and monitor their patients remotely. The wide variety of connected measuring equipment and questionnaires offers healthcare providers the opportunity to digitise the many different care paths of their (chronically ill) patients. Care paths can be easily assigned to a patient and from then on the Emma Platform monitors the progress.
The Emma Platform ensures that the equipments is sent to the patient and that the patient is supported in taking the measurements at the agreed moments. This way, care providers can digitise the care they provide, without any additional administrative burdens.
